Why do I start getting very hot and dizzy and at time pass out?

I am 20 years old, I start getting very hot on my head then feel as tho i cant breathe. My shoulders start to hurt and my hands start to swell and my fingers feel like sausages, I have passed out a few times, and my heart is racing - this will last for 15- 30 minutes.
